How do I make an appointment?
Please call us on 07 32615125 to speak to one of our receptionists to make an appointment.
This way we can make sure any questions you have can be answered and you will have all the information you need. Please try to arrive at least 5 minutes before your appointment time to fill in any paperwork.
What should I bring?
It is helpful to bring your reading glasses if you use them.
If you have a referral, medical certificates, surgical discharge notes, xrays, scans or ultrasounds you should bring those along. Please wear or bring along clothes that allow the physiotherapist to be able to examine the area you are needing treatment for but don’t worry if you forget we have shorts and singlet tops you can use.
If you have private health insurance bring your membership card.
If you are eligible to claim a rebate from Medicare bring your Medicare card and a debit card.
Do you send reminders for appointments?
We try to send reminders of appointments a day or two before- please let us know by phone on 07 3261 5125 if you are unable to keep your appointment- fees may apply for non-attendance and last minute cancellations.
What paperwork do I need, do I need a referral ?
Referrals are always welcome but are not necessary if you are coming as a private patient. Bring your health fund card if you want to make a claim.
You will need a referral if you are coming for assessment or treatment under the Department of Veterans Affairs, Enhanced Primary Care Medicare Program or Injurynet Program.
Been injured at work?
If you have had a work related injury and are seeking treatment through Workcover Queensland you will need a current medical certificate and an accepted claim. We have a GAP fee payable at each consultation for Workcover patients.
Been in a car accident?
If you have been involved in a car accident and are seeking treatment through CTP Insurance you need a treatment plan approved before we can send accounts to the insurance company direct- if you have an authorisation letter please bring that along. Many people start treatment straight away and seek reimbursement for their paid fees from the insurer once the treatment plan is approved.
How do I pay?
Private patients – If you are coming as a private patient you can claim from your health insurance via HICAPS and settle the difference with cash, credit card or EFTPOS on the day of treatment.
DVA – We can direct bill to DVA.
Injurynet Progam – We can send accounts direct to participating employers for approved claims.
Enhanced Primary care Program – Medicare is NOT bulk-billed. Patients referred under this program pay our normal fees first and then if they have their Medicare card and a DEBIT card we can process the rebate from Medicare via easyclaim. There is an out of pocket amount. Rebates can also be claimed via Medicare offices if easyclaim is unavailable.
Workcover – Gap fees are payable at each visit. If a claim is rejected- all fees are payable by the patient.
CTP claims – we can send accounts to your insurer once our treatment plan and fees have been accepted.
